in-bore = biopsja
in gantry. Gantra czyli dziura, w której się leży podczas badania MR.
Biopsja in -gantry może być przezkroczowa (
transperineal) lub transrektalna, w zależności od stosowanego systemu pobierania biopsji.
Podczas biopsji in-bore zazwyczaj pobiera się wycinki tylko ze zmiany wskazanej w MRI (wystarczają 3).
20-30 wycinków, to jest już biopsja saturacyjna, którą wykonuje się w przypadku gdy wszystko wskazuje na obecność raka, a biopsje są negatywne.
----------EDIT (06.07.2023) ----------Obecnie są stosowane 3 typy biopsji prostaty z udziałem MRI:
=> biopsja "fuzyjna" kognitywna - urolog "w umyśle" nakłada wcześnie uzyskany obraz MR na obraz USG uzyskiwany w czasie realnym;
=> biopsja fuzyjna z użyciem urządzenia, które umożliwia nakładanie wcześniej uzyskanego obrazu MRI na obraz USG uzyskiwany w czasie realnym;
=> biopsja pod kontrolą MR in-bore (obecnie najczęściej robotyczna).
Biopsja kognitywna ma szereg negatywów - brak standardów dot. jej przeprowadzania, całkowicie zależność od umiejętności operatora,brak możliwości potwierdzenia lokalizacji igły biopsyjnej oraz słaba efektywność jeśli chodzi o małe zmiany.
Badania sugerują wyższość biopsji in-bore nad biopsją fuzyjną, ale biopsja in-bore jest mniej dostępna, droższa i bardziej czasochłonna niż biopsja fuzyjna. Z tego powodu biopsja in-bore, jeśli dostępna, jest najczęściej używana do rozwiązywania tzw. przypadków trudnych.

Consensus Statements
• MRI-TB reveals more csPCa and less clinically insignificant PCa than systematic biopsy and is becoming (or has become) the standard of practice in diagnosing PCa.
• The three common MRI-TB techniques are cognitive biopsy, fusion biopsy, and MRI in-bore biopsy. The optimal MRI-TB technique has yet to be determined.
However, cognitive biopsy has key disadvantages, which include operator dependence, lack of a standardized approach, inability to confirm needle location, and reduced effectiveness for small lesions. Thus, fusion biopsy or MRI in-bore targeted biopsy, when available, is usually preferred over cognitive biopsy.
• Results of recent studies suggest better performance of MRI in-bore biopsy than of fusion biopsy. However, MRI in-bore biopsy is less widely available and is more expensive to implement and more time-consuming than fusion biopsy. Thus, MRI in-bore biopsy, when available, may be reserved for problem-solving or difficult cases.
• Fusion biopsy is a good option because it is less expensive and has less impact on workflow than does MRI in-bore biopsy. Current limited evidence suggests that fusion biopsy, compared with cognitive biopsy, may be better for targeting lesions that are not visible on US and may be better for detecting small lesions and lesions in the transition zone.
• Cognitive biopsy is the least expensive of the techniques and easiest to implement but does not allow recording of the biopsy site for future reference. For centers starting to perform MRI-TB, cognitive biopsy remains a good option when the other techniques are unavailable.
• All three MRI-TB, techniques can be performed by a transrectal or transperineal approach. The transrectal approach is more widely used but is associated with risk of sepsis. The transperineal approach has low risk of infection and is increasing in use. The transperineal approach can be performed with local anesthesia only.
