Przedwczoraj w mediach bardzo zachwalano test diagnostyczny 4Kscore®Test, wykonywany przez jedną z prywatnych placówek k. Warszawy. NFZ nie refunduje, koszt ok 2000zł. Czy ktoś korzystał, czy warto (np. w mojej sytuacji)? a może już ten temat był na forum? ja szukałem bez powodzenia

Byl tylko link do artykulu reklamujacego firme dr F., ale zostal skasowany z wiadomych powodow.

Wg mojej najlepszej wiedzy nikt z piszacych na forum nie zafundowal sobie tego testu.
W Twojej specyficznej sytuacji zastanowilabym sie nad zainwestowaniem 1600 zl, ale dopiero po przedyskutowaniu pros & cons z niezainteresowanym zyskiem dobrym specjalista.

What is the 4Kscore Test?

The 4Kscore Test helps clarify the biopsy decision-making process by determining a patient specific probability for finding aggressive, Gleason score 7 or higher prostate cancer upon biopsy. These are the aggressive prostate cancers that always require medical treatment or intervention.

The 4Kscore Test relies on the measurement of four prostate-specific kallikreins in the blood:
Total PSA, Free PSA, Intact PSA, and Human Kallikrein 2 (hK2).
The blood test results are combined in an algorithm with patient age, digital rectal exam (nodules, no nodules), and prior negative biopsy (yes, no) to give physicians a personal score for each patient.

The 4Kscore Test predicts the risk percent score from <1% to >95% of a man having aggressive cancer in a prospective biopsy.


PSA testing on its own has been shown to result in a high number of unnecessary biopsies. By providing information to improve decision making before ordering a prostate biopsy, the 4Kscore Test can help avoid an unnecessary prostate biopsy and its associated complications, and costs for men at low risk of having an aggressive cancer, while still identifying men at high-risk for having an aggressive prostate cancer.
Depending on the 4Kscore result, Urologists can choose to either place a low-risk patient under active monitoring, or perform a biopsy on a high-risk patient.

The 4Kscore Test has undergone extensive clinical research and laboratory test validation.
The biomarkers utilized in the 4Kscore Test are based on over a decade of research conducted by scientists at the Memorial Sloan-Kettering Cancer Center and leading research centers in Europe, encompassing over 10,000 patients.
The results have recently been replicated in a prospective blinded clinical study conducted at 26 urology centers across the United States on 1,012 patients.
Based on the results of this clinical study, as many as 30-58% of biopsies are avoidable using the 4Kscore Test.



Who Needs the 4Kscore Test?

The 4Kscore Test is for any man concerned about his risk for high-grade, aggressive prostate cancer, and is especially useful when considering a prostate biopsy. The 4Kscore test provides information that a man and his Urologist can use to make an informed, shared decision about whether or not to perform a prostate biopsy.


Common Reasons for a TRUS prostate biopsy (Transrectal Ultrasound):

=> Family history of prostate cancer
=> Elevated PSA or High PSA
=> Abnormal results from a digital rectal exam (DRE)

What is the benefit of the 4Kscore Test?

The 4Kscore Test can help reduce the number of unnecessary biopsies, by measuring a man’s risk of having aggressive prostate cancer.
If that risk is low, a patient may, in consultation with his Urologist, decide not to have a prostate biopsy and avoid the associated pain and complications such as bleeding and infection that may result from the biopsy procedure.
Also, by avoiding a diagnosis of a low-grade, non-life threatening form of prostate cancers (Gleason score = 6), the patient may avoid treatments that he may not need.

According to data from a large, prospective, multi-institutional trial carried out here in the USA, the 4KScore test has shown a high degree of ability to accurately identify men with a Gleason score of 3 + 4 = 7 or higher which was subsequently confirmed on prostate biopsy.

The paper by Punnen et al. was another of those presented at the ongoing Genitourinary Cancers Symposium in Orlando, Florida, and offers strong evidence that, for many men, an initial 4KScore test (after a prior PSA test suggests possible risk for prostate cancer) may be wiser than an immediate biopsy — assuming that there are no other clear signals of prostate cancer risk.
Additional information about the study is also available in a report on the UroToday web site.

As some readers will be aware, the 4KScore test uses data from a standard blood sample that combines information from four different kallikrein assays (total PSA, free PSA, intact PSA, and hK2), along with the patient’s age, his rectal exam result, and the results of any prior biopsy, to create an actual risk “score”.

In the trial discussed here, Punnen et al. used the 4KScore results for 1,012 men who were undergoing risk assessment at 26 different institutions in the US between October 2013 and April 2014.
The goal was to determine whether each individual patient was likely to have a positive prostate biopsy with a Gleason score of 3 + 4 = 7 or higher.
All patients subsequently underwent a systematic TRUS-guided biopsy, regardless of their PSA level or any other clinical findings.

What Punnen and his colleagues report is, basically, the following:

781/1,012 men (77.2 percent) had either a negative biopsy result or a positive biopsy result with a Gleason score of 6.

231/1,012 men (22.8 percent) had a positive biopsy result with a Gleason score of 3 + 4 = 7 or higher, among whom:

- 23.0 percent (i.e., 10.7 percent of all men biopsied) were Gleason 3 + 4 = 7.
- 12.8 percent (i.e., 5.8 percent of all men biopsied) were Gleason 4 + 3 = 7.
- 7.4 percent (i.e., 3.5 percent of all men biopsied) were Gleason 8.
- 5.5 percent (i.e., 2.6 percent of all men biopsied) were Gleason 9.
- 0.6 percent (i.e., 0.3 percent of all men biopsied) were Gleason 10.

The 4KScore was able, prospectively, to predict the probability of a Gleason score of 3 + 4 = 7 with an extremely high degree of accuracy.
The 4KScore was significantly more accurate than the PCPT risk calculator in predicting risk for a Gleason score of 3 + 4 = 7 or higher (p < 0.0001).

If it is assumed that a 9 percent probability of cancer with a Gleason score of 3 + 4 = 7 is a reasonable threshold for biopsy of the prostate, then use of the 4KScore in this cohort of patients would have:

- Avoided unnecessary biopsies in 434/1,012 men (43 percent)
- Led to a delay in diagnosis of 24/231 cancers (10.4 percent) with a Gleason score of 3 + 4 = 7 or higher


Punnen et al. conclude that:
The 4Kscore demonstrated excellent accuracy in detecting high-grade prostate cancer.
It is a useful tool in selecting men who are likely to have high-grade disease and most likely to benefit from a prostate biopsy versus those men with no cancer or indolent cancer.

It will be interesting to see whether the National Comprehensive Cancer Network’s guidelines committee — and/or the American Urological Association’s comparable committees — are willing to accept these prospective data as sufficient evidence to recommend a role for the 4KScore test in the work-up of a patient with an elevated PSA level prior to implementation of prostate biopsy.

What is not reported in this study are:
- the number of men who had a Gleason 6 biopsy result;
- the number of men who had a negative biopsy result;
- and the accuracy of the 4KScore in differentiating between these two categories of patients.

The degree to which the absence of this information may affect the value of the 4KScore test is likely to be debatable depending on one’s point of view about the necessity for early diagnosis of indolent and low-risk forms of prostate cancer.

This confirms the findings of an earlier prospective trial by Lin et al. It’s always good to have confirmation from independent trials.
I think the 4KScore is a potentially valuable tool for detecting the kind of prostate cancer we most want to detect, and eliminating unnecessary biopsies.
The only problem is the cost — $395 is steep!
By comparison, the Prostate Health Index (PHI), was found to be similarly effective in a comparative study published by Nordstöm et al. last year.
PHI only costs $80 and is guaranteed to be covered by insurance. Between the two, I’d go for PHI.

“PHI and 4Kscore tests provide somewhat different information.
As a result, one is not better than another.
Rather they provide complementary information.
While either test is useful by itself, together they provide a fuller picture of prostate cancer risk for individual man.
Prostate Health Index (PHI) test predicts the risk of having prostate cancer.
4KScore test predicts the risk of having high-risk prostate cancer.

Because prostate cancer is an evolving disease these tests together may provide a more-rounded picture about longer-term risk of aggressive prostate a cancer in men.”

We want to be careful about quoting opinions of some urology group and taking it as fact.
While it’s true that the 4KScore is better at predicting higher grade cancers, as shown in the Lin et al. study I linked in my reply above, it is also true that PHI seems to be better at predicting higher grade, higher stage, and higher volume cancers at least in one small scale study published by Ferro et al.

Note the following from David Okrongly — President, Diagnostics Division:

“We’re getting ready to begin a very large study in active surveillance using a cohort that has been followed for five years now under active surveillance.
Active surveillance is when a man is diagnosed with Gleason 6 or indolent cancer, they go onto a program of repeated PSA testing and biopsying and we think that the 4Kscore test should provide exceptional clinical value in awarding biopsies during active surveillance.”

dzięki za odpowiedź, zasada "to consider the pros and cons" jest ze wszech miar słuszna. Korci mnie także test PCA3, ale nie chciałbym być durnym sponsorem kolejnej nowej wypasionej fury jakiegoś doktora. Lecz jesli warto, to nie ma ceny za zycie i zdrowie

@ tab:
Jak na razie chyba jestem jedyną osobą na naszym forum, która zafundowała sobie test PCA3. I nie żałuję, gdyż skrócił on znacznie moją diagnostykę i ukierunkował na dalsze działania. Gdyby nie ten test i jego wynik, pewnie dalej bym się "bujał" z dziwnymi działaniami takimi jak podawanie finasterydów w celu sprawdzenia czy czasem PSA nie jest spowodowane przerostem prostaty, albo zwykłymi biopsjami,których trafność jest na dość niskim poziomie.

Co do "4Kscore(R)Test", to w odróżnieniu od PCA3 nie jest to jeszcze tak samo rozpowszechnione badanie i chyba raczej nie ma zbyt wiele prac na temat trafności tego testu. Przynajmniej jak robiłem swego czasu kwerendę na ten temat to nie natrafiłem na zbyt wiele opracowań.

Zosiu , z artykułu Cancerifnfolink wnioskuję, że 4Kscore test jeszcze jest w fazie dużej dyskusji. O ile wiem, to fakt wystąpienia raka jednoznacznie może być stwierdzony tylko przez histopatologa ( bezpośrednio ma zobaczyć tkankę neo). I to dopiero determinować może podjęcie leczenia. Wynik tesu 4Kscore to tylko sugestia do dalszych poszukiwań. Trochę galimatias...

The 4Kscore Test measures the plasma levels of four different prostate-derived kallikrein proteins:
- Total PSA,

What can the 4Kscore Test do for my practice?
The 4Kscore Test can help urologists and physicians to identify patients who can benefit from biopsy while avoiding unnecessary biopsy for low-risk patients. The result is that more of the right patients get the diagnosis and treatment they need, while unnecessary biopsy is minimized.


What can't the 4Kscore Test do?

The 4Kscore Test does not provide a diagnosis of prostate cancer. Rather, it is designed to help clarify the decision on whether or not to perform a biopsy based on the probability of a patient having aggressive prostate cancer.


What do I do with an elevated 4Kscore Test result?

The 4Kscore Test provides information on the risk (probability) of a man having aggressive prostate cancer (Gleason score 7 or higher). Other clinical information may modify an individual patient’s risk and the decision to perform a prostate biopsy should be an individual choice based upon shared decision making between Urologist and patient.


What are the limitations or patient restrictions for the 4Kscore Test?

=> With a previous diagnosis of prostate cancer
=> That has received a DRE in the previous 96 hours (4 days) before phlebotomy. A DRE performed after the phlebotomy is acceptable
=> That has received 5-alpha reductase inhibitor (5-ARI) therapy, such as Avodart® (dutasteride) or Proscar® (finasteride), within the previous six (6) months
=> That has undergone any procedure or therapy to treat symptomatic BPH or any invasive, urologic procedure that may be associated with a secondary PSA elevation prior to phlebotomy within the previous six (6) months

Badanie 4Kscore®

Ponieważ dział "Mam pytanie" jest zablokowany, umieszczam moje pytanie tutaj. Jeśli to jest miejsce nieodpowiednie, proszę adminów o przeniesienie.

Ze względu na obciążenia dziedziczne raka prostaty, nakłoniłem mojego 40-letniego syna na zrobienie badania PSA.
W badaniu sprzed pół roku PSA wyszło 1,52 ng/ml, natomiast teraz 1,73 ng/ml.
Niepokój zasiany, lepiej dmuchać na zimne, stąd moje pytanie/prośba o rady w wybraniu ścieżki diagnostycznej.
Co sądzicie o badaniu 4K? To badanie nazywa się 4Kscore® -zb

4K to test lepiej sprawdzający się w przypadku raka złośliwego z GS>7. No i też PSA powinno być wyższe, aby wynik był bardziej prawdopodobny.

Nie wiem, czy nie lepiej zrobić test komórek krążących CTC na bazie testów genetycznych, czyli z wykrywaniem DNA komórek rakowych.

Test 4Kscore jest przeznaczony dla pacjentow przed diagnozą CaP.
Więc ja bym raczej napisala, że ten test określa ryzyko wystąpienia agresywnego raka.
