Prostate imaging-reporting and data system (PI-RADS)

PI-RADS which an acronym for Prostate Imaging-Reporting and Data System refers to a structured reporting scheme for prostate cancer.

The original PI-RADS score was annotated, revised and published in it second version PI-RADSv26.
The following article reflects the newest revision.

The score is assessed on prostate MRI.
Images are obtained using a multi-parametric technique including T2 weighted images, a dynamic contrast study (DCE), DWI, and possibly spectroscopy (MRS).


A score is given according to each variable.
The scale is based on a score from 1 to 5 (which is given for each lesion), with 1 being most probably benign and 5 being highly suspicious of malignancy.



These parameters include:

T2 signal

for the peripheral zone (PZ)
a. uniform high signal intensity (SI) - 1 (point)
b. linear, wedge shaped, or geographic areas of lower SI, usually not well demarcated - 2
c. intermediate appearances not in categories a/b or d/e -3
d. discrete, homogeneous low signal focus/mass confined to the prostate - 4
e. discrete, homogeneous low signal intensity focus with extracapsular extension/invasive behaviour or mass effect on the capsule (bulging), or broad (>1.5 cm) contact with the surface - 5

for the transition zone (TZ)
a. heterogeneous TZ adenoma with well-defined margins: “organised chaos” - 1
b. areas of more homogeneous low SI, however well marginated, originating from the TZ/BPH - 2
c. intermediate appearances not in categories a/b or d/e
d. areas of more homogeneous low SI, ill defined: “erased charcoal sign” - 4
e. same as d, but involving the anterior fibromuscular stroma or the anterior horn of the PZ, usually lenticular or water-drop shaped - 5

Diffusion weighted imaging (DWI)
a. no reduction in ADC compared with normal glandular tissue. No increase in SI on any high b-value image (≥b800) - 1
b. diffuse, hyperintensity on ≥b800 image with low ADC; no focal features, however, linear, triangular or geographical features are allowed - 2
c. intermediate appearances not in categories a/b or d/e - 3
d. focal area(s) of reduced ADC but isointense SI on high b-value images (≥b800) - 4
e. focal area/mass of hyperintensity on the high b-value images (≥b800) with reduced ADC - 5


Dynamic Contrast Enhancement (DCE)-MRI
type 1 enhancement curve: 1
type 2 enhancement curve: 2
type 3 enhancement curve: 3
+1 point for for focal enhancing lesion with curve type 2–3
+1 point for asymmetric lesion or lesion at an unusual place with curve type 2–3


Additional scoring can be done with MR spectroscopy for a 1.5T scanner using the citrate and choline peak (3 voxels)
citrate peak height exceeds choline peak height >2 times - 1 point
citrate peak height exceeds choline peak height times >1, <2 times - 2 points
choline peak height equals citrate peak height - 3 points
choline peak height exceeds citrate peak height >1, <2 times - 4 points
choline peak height exceeds citrate peak height >2 times - 5 points
The lesions are then graded according to a classification system.


PI-RADS I - most probably benign -
total score with T2, DCE, DWI = 3,4
total score with T2, DCE, DCE and MRS = 4,5

PI-RADS II - probably benign
total score with T2, DCE, DWI = 5,6
total score with T2, DCE, DCE and MRS = 6-8

PI-RADS III - indeterminate
total score with T2, DCE, DWI = 7-9
total score with T2, DCE, DCE and MRS = 9-12

PI-RADS IV - probably malignant
total score with T2, DCE, DWI = 10-12
total score with T2, DCE, DCE and MRS = 13-16

PI-RADS V - highly suspicious of malignancy
total score with T2, DCE, DWI = 13-15
total score with T2, DCE, DCE and MRS = 17-20


In addition to the PI-RADS score for the probability of a lesion to be significant, extra-prostatic involvement should also be scored on a five-point scale 2.

Multi-parametric prostate and whole-body oncology MRI exams

UCLH uses Ingenia 3.0T for multi-parametric MRI exams to help localize and characterize lesions[/i]
Not using an endorectal coil is more comfortable for the patient and currently the surface coil approach provides enough SNR for prostate imaging. When performing prostate MRI without endorectal coils, air in the rectum may cause susceptibility effects, particularly at 3.0T. So, in order to reduce the susceptibility effect, we use a small field-of- view DWI for generating the ADC map. We also include a separate high b-value DWI and have found both particularly useful. When I see focal areas of high signal on b2000 images and the rest of the gland is dark, I have a very high suspicion of cancer being present.
We have been performing multi-parametric MRI of the prostate without an endorectal coil at 1.5T since 2006. The arrival of our Achieva
3.0T TX system and use of the 32-channel SENSE Cardiac coil significantly improved the signal to noise of our T2 and diffusion weighted images.

MRI in addition to or as a substitute for prostate biopsy: The clinician's point of view

Pre-biopsy mp-MRI improves identification and characterisation of cancer of the prostate
Mp-MRI is effective for studying the anterior and posterior regions of the prostate in patients presenting lesions suspected of being neoplastic.

A European consensus group of MRI experts recently recommended that this modern imaging technique should be performed as an examination prior to biopsy [1].
It is however necessary for the MRI to be performed in a standard way in all centres.


The experts agreed on 67% of the 260 items related to the imaging sequences necessary, and decided for example, that the MRI sequences necessary are T2-weighted, dynamic T1 contrast-enhanced and diffusion-weighted sequences, whereas spectroscopy is not recommended.
The experts also agreed on 54% of the 260 issues concerning interpreting the images.
At least 16 and up to 27 prostate zones where cancer may occur must be described (Fig. 1.)

A suspected cancer scale from 1 to 5 was approved to harmonise communication in describing the lesions observed.

Because of increase in the signal-to-noise ratio, 3 Tesla MRI decidedly improves spatial, temporal and spectral resolution of the prostate in all imaging sequences.
Nevertheless, 1.5 Tesla with a pelvic coil can be used to provide interpretation of the signals observed in suspect areas that is similar to 3 T MRI.
It has, indeed, been decided that this use of 1.5 T MRI with a pelvic coil is adequate for standard clinical practice.
An endorectal coil is no longer necessary for performing prostate MRI to detect cancer.

The score is assessed on prostate MRI. Images are obtained using a multi-parametric technique including T2 weighted images, a dynamic contrast study (DCE) and DWI. If DCE or DWI are insufficient for interpretation the newest guidelines recommend omitting them in the scoring6.

A score is given according to each variable. The scale is based on a score from 1 to 5 (which is given for each lesion), with 1 being most probably benign and 5 being highly suspicious of malignancy:

PI-RADS 1 - Very low (clinically significant cancer is highly unlikely to be present)

PI-RADS 2 - Low (clinically significant cancer is unlikely to be present)

PI-RADS 3 - Intermediate (the presence of clinically significant cancer is equivocal)

PI-RADS 4 - High (clinically significant cancer is likely to be present)

PI-RADS 5 - Very high (clinically significant cancer is highly likely to be present)

These parameters include:

T2-weighted imaging

Peripheral zone (PZ)

1 - uniform high signal intensity (SI)

2 - linear or wedge shaped hypointensity or diffuse mild hypointensity, usually indistinct margin

3 - heterogeneous signal intensity or non-circumscribed, rounded, moderate hypointensity

4 - circumscribed, homogenous moderate hypointense focus or mass confined to prostate and <1.5cm in the greatest dimension

5 - same as 4 but ≥1.5 cm in greatest dimension or definite extraprostatic extension/invasive behavior

Transition zone (TZ)

1 - Homogenous intermediate signal intensity

2 - Circumscribed hypointense or heterogenous encapsulated nodule(s) (BPH)

3 - Heterogeneous signal intensity with obscured margins. Includes others that do no qualify as 2, 4, or 5

4 - Lenticular or non-circumscribed, homogenous moderately hypointense, and <1.5cm in greatest dimension

5 - Same as 4, but ≥1.5cm in greatest dimension or definite extraprostatic extension/invasive behavior

Diffusion weighted imaging (DWI)

1 - no abnormality and ADC and high b-value DWI

2 - indistinct hypointense on ADC

3 - focal mildly/moderately hypointense on ADC and isointense/mildly hyperintense on high b-value DWI

4 - focal markedly hypointense on ADC hyperintense on high b-value DWI; <1.5cm in greatest dimension

5 - same as 4 but ≥1.5cm in greatest dimension or definite extraprostatic extension/invasive behavior

Dynamic Contrast Enhancement (DCE)-MRI

Negative (-) - no early enhancement, or diffuse enhancement not corresponding to a focal finding on T2 and/or DWI or focal enhancement corresponding to a lesion demonstrating features of BPH on T2

Positive (+) - focal, and earlier than or contemporaneously with enhancement of adjacent normal prostatic tissues, and corresponds to suspicious finding on T2 and/or DWI

Currently, MR spectroscopy is not included in PIRADS scoring.

Gdzie w Polsce dobrze wykonuja i opisuja mp-MRI?

Czy w takim razie mozesz zaprezentowac opis swojego badania mp-MRI?
I napisac w jakich okolicznosciach bylo robione.

Badanie to zależy od dwóch czynników - dobrego sprzętu i doświadczenia osoby opisującej wyniki.
Dobrym specjalistą jest wg dr Sosnowskiego pani prof. Tacikowska. Badanie przez nią opisywane wykonywane jest w LUX-med w Warszawie. Niestety, ich słabością jest aparat - wyłącznie1,5 T. Dużo lepszy sprzęt (nowy, 3-teslowy) ma Voxel przy Szpitalu Wojskowym w Krakowie, mają też niezły zespół odczytujący.

Naukowcy z University College London Hospitals sugerują, że multi-parametryczne badanie rezonansem magnetycznym prostaty (mpMRI) powinno być zastosowane przed wykonaniem pierwszej biopsji.
My, tu na forum, już od jakiegoś czasu podajemy te informacje w postach.

Tak, to prawda.
Coraz wiecej osrodkow na swiecie dziala w ten sposob.

Natomiast co do praktyki - wazne, zeby byl aparat 3T, ale takze radiolog, ktory wie o co chodzi (sam sprzet nie jest gwarancja)

Gdyby ktos potrzebowal powyzsza solidna konstelacje SPRZET 3T MR+RADIOLOG (znajacy PI-RADS i wspolpracujacy z urologami) w Dolnoslaskim (Wroclaw), to mam dobry adres.
Wiadomosc na priva, zeby nie uprawiac reklamy.

Od lat zajmuje się tematyką Aging Male (problemy starzenia się u mężczyzn) (Männergesundheit, MensHealth) w organizacjach temu poświęconych. Chodzi tu o całościowy aspekt zdrowotny u facetów - łącznie z psychiką. Jednym z tematów jest także testosteron oraz ED u mężczyzn ...od 40-go roku życia.

Ad rem.
Jak ja to robię.
U Pacjenta, który kwalifikuje się do 3T mpMRI robię dodatkowo elastografię prostaty. Po co? Po pierwsze przydaje mi się to do porównania potem z mpMRI, a po drugie mpMRI jest bardzo drogie i czasami elastografia prostaty bywa jedynym badaniem przed biopsją (a jest mimo wszystko dokładniejsza niż zwykły TRUS). Jeśli Pacjent decyduje się na mpMRI, kontaktuję się z radiologiem z doświadczeniem oceny PIRADS vs2, któremu opisuję przypadek i wpisuję to na skierowaniu żeby pamiętał rozmowę.
Potem raz jeszcze dajemy zarobić telekomunikacji i omawiamy szczegóły i wątpliwości. Na razie to funkcjonuje. Dobre jest to, że uczymy się nawzajem i tu tkwi, moim zdaniem w każdej z dziedzin, klucz do sukcesu... kiedyś pozwolę sobie wkleić kilka dokumentów z powyższego przebiegu.
