Optymalna liczba cykli Docetakselu w mCRPC

Co do terapii Dx, to czytałem opracowanie, z ktorego wynikało, że liczba podanych wlewów determinowała przeżycie (OS) chorych z CaP.
Wyraźnie od 8 wlewu Dx w górę. Lepiej jeszcze chyba było od 10-12.
Fakt, że to wydłużenie życia, to były miesiące a nie lata. Ale zawsze to coś. Ale niestety nie pamiętam czy musiała być pozytywna odpowiedź czyli spadek PSA?

Tu masz badanie, które mówi, że dokładnie nie jest tak jak piszesz. Więcej wlewów wcale nie oznaczało dłuższego czasu przeżycia, a tylko zwiększało liczbę niepożądanych efektów. Niestety nie była to duża grupa badanych.

The study included 33 patients receiving ≤ 10 cycles of docetaxel and 40 patients receiving > 10 cycles. Patients receiving > 10 cycles were younger than those who received ≤ 10 cycles. There was no statistical significant difference in overall survival between the two groups (log-rank test, p = 0.75). Adverse effects were more common among patients receiving ≥ 10 cycles of treatment.

W badaniu podawano w ramieniu A docetaksel, prednizon i lenalinomid, a w ramieniu B docetaksel, prednizon i placebo. Badanie przerwano, kiedy okazało się, że skutki uboczne kombinacji z lenalinomidem są poważne. Ze względu na toksyczność, w ramieniu A podano przeciętnie 6 cykli docetakselu, w drugim ta liczba była większa.
W ramach analizy post hoc stwierdzono, że więcej cykli docetakselu dawało lepszą przeżywalność. Jeśli znasz się na statystyce, możesz przeanalizować wywody autorów, ale dane te nie pochodzą z badania zaprojektowanego na porównanie skuteczności leczenia/długości przeżycia w zależności od ilości cykli docetakselu.

Timing of PSA response to guide cessation of docetaxel in prostate cancer. (2013)

Abstract

Background:
Docetaxel plus prednisolone is an established treatment for men with advanced prostate cancer. Clinicians need to be able to identify men who are not benefiting from chemotherapy, and a key unanswered question is how many cycles of docetaxel should men receive before this assessment is made. We investigated the prognostic significance of a PSA response to docetaxel chemotherapy and the number of cycles after which an absence of PSA response could reliably predict future non-response.

Methods:
Data was collected for men who received one to ten cycles of docetaxel for castrate refractory prostate cancer between 2005 and 2011. PSA was monitored during treatment. All men were treated at a single centre, The Christie NHS Foundation Trust, UK. Survival is defined as the interval between the first dose of docetaxel and the date of death.

Results:
Data was available for 320 patients. At the commencement of chemotherapy, median age was 67 years (range 49-81yrs) and median PSA was 207ng/ml. All patients had a Karnofsky Performance Status ≥ 70%. There were 239 deaths and the median survival of the whole group was 403 days. 140 (44%) patients received 6 cycles of docetaxel and 23 patients (7%) received 10 cycles. Median survival was significantly longer in men who had any fall in PSA during chemotherapy compared to those whose PSA did not fall (462 days vs 268 days, p<0.001). Any PSA fall occurred during the first 4 cycles of docetaxel for 95.1% of men.
Median survival was significantly longer in men who had a 50% fall in PSA during chemotherapy compared to those who did not (491 vs 357 days, p<0.001).
The 50% fall in PSA occurred during the first 5/6 cycles of chemotherapy in 91.5% and 97.7% of men respectively. [b]

Conclusions:
A patient’s clinical condition is of paramount importance when assessing them during docetaxel chemotherapy.
Our data suggest that a fall in PSA following docetaxel is an indicator of improved overall survival.
PSA response could help clinicians decide when to stop docetaxel; patients are unlikely to have a fall in PSA if it has not occurred during the first 4 cycles of chemotherapy and they fall into a poor prognostic group. These patients should be considered for alternative treatment in order to improve outcomes and spare them unnecessary toxicity.

Currently, there are several life-prolonging therapies available for patients with mCRPC, but treatment selection is largely based on clinical factors and no validated predictive biomarkers are yet available to guide therapeutic choices.

The mechanisms of escape to novel androgen-receptor signaling (ARS) inhibitors are now better understood and include ligand-dependent and -independent mechanisms.

mCRPC remains a highly heterogeneous disease and many prognostic biomarkers have been identified to help estimate survival in this setting, such as lactate dehydrogenase (LDH), hemoglobin, ECOG performance status, PSA, albumin, and others.

Recently, CTC count and AR-V7 status have been described as prognostic markers. The presence of a baseline CTC count ≥5 (and post-treatment CTC conversion to <5) and/or AR-V7 detection are associated with overall worse survival.

There have been significant advances to identify biomarkers for therapy selection, such as AR-V7 status, AR mutations, CTC heterogeneity, the presence of DNA damage repair gene alterations. Prospective validation of these biomarkers is still pending prior to incorporation in routine clinical use.

AR-V7 is a constitutionally active form of the AR and has been implicated in primary and acquired resistance to ARS inhibitors such abiraterone and enzalutamide, but is compatible with sensitivity to taxane chemotherapies.

Patients with AR-V7-positive CTCs may still respond to taxane-based chemotherapy, including some cases that are associated with AR-V7 conversions from positive to negative. Therefore, if these findings are confirmed in validation studies, AR-V7 status may serve as a treatment-selection tool for patients with mCRPC.

Tak, postarałem się znależć "to coś" i to rzeczywiście był artykuł z cytowaniem z Mainsail Study!

W tłumaczeniu:
Pacjenci, którzy otrzymali osiem lub więcej cykli leczenia, mieli poprawę w OS niezależnie od leczenia lenalidomidem. Również pacjenci, którzy otrzymali więcej niż 10 cykli, mieli medianę OS 33 miesiący w porównaniu z 26,9 miesiąca w przypadku pacjentów otrzymujących 8 do 10 cykli i 22,8 miesięcy w przypadku pacjentów otrzymujących 5 do 7 cykli (P <0,001).

Rozumiem, że jak zwykle brak dużych prospektywnych, dobrze skonstruowanych, randomizowanych badań klinicznych a praktyka dnia codziennego zależy od lekarza prowadzącego, jego wiedzy i nosa oraz preferencji chorego :/

Dyskutujemy co można zrobić, ale każdy chory musi wiedzieć, że nie zawsze białe jest białe a czarne - czarne.
Docetaxel wydawał mi się "niegroźny" a ryzyko objawów ubocznych "przereklamowane", ale od czasu kiedy zacząłem mieć polineuropatię konczyn dolnych po ChT i wiem jakie to upierdliwe, uważam, że każdy kto to bierze ChT musi mieć właściwie ustawiony stosunek ryzyka do korzyści.

Po wizycie u p. dr Skonecznej jeszcze jedna impresja. P. dr stwierdziła, że ona w moim przypadku zastosowałaby tylko 6, a nie 8 czy więcej wlewów, choćby po to, żeby spróbować uniknąć toksycznego działania Docetaxelu. A w moim przypadku, żeby próbować uniknąć neuropatii.
A skąd wiedziałaby, że akurat u Ciebie moze wystąpic neuropatia? Wszak nie masz cukrzycy? -zb

Wyraziła tez pogląd, że takie powikłania zdarzają się coraz częściej od kiedy zamiast oryginalnego leku zaczęto stosować leki generyczne, które mają bardziej nasilone działanie toksyczne.

Toxicities of docetaxel: original drug versus generics—a comparative study about 81 cases


Several theories have attempted to explain the toxicities with generic docetaxel based on pharmacological characteristics.
The mechanisms involved were underdosing docetaxel, high level of impurities and excipients used.


Levels of docetaxel were evaluated by Vial et al. on generic formulations purchased in 14 countries (Vial et al. 2008).
The results were surprising - 21 generics contained less than 90 % docetaxel, 11 of which had less than 80 % of what would be expected.
Only 10 were in the acceptable range of 90–110 %.

Levels of impurities were also obtained in same study using reverse-phase liquid chromatography with ultraviolet detection setting a conservative limit of 3 % (the reference was 1.6 %). They found that 23 of the generics had impurities levels of more than 3 % and one of the generics, from India, had a 20 % level of impurities. Importantly, 33 of the impurities were not detected at all in the reference which that we do not know the physiological role and clinical consequences. In our study, the first and the third generic employed have high levels impurities (which one had a more than 6 %).

The role of excipients is also reported, docetaxel is a hydrophobic drug that requires solvents to improve its solubility and allow its constitution.
The solubiliser used is polysorbate 80 (Tween 80), a non-ionic surfactant vehicle whose main constituent is polyoxyethylene-20-sorbitanmonooleate which is structurally equivalent to polyethylene glycols (PEG).
Nevertheles, polysorbate 80 is a pharmacologically and biologically active excipient associated with side effects such as hypersensitivity reactions, peripheral neuropathy (Tije et al. 2003), vascular toxicity and fluid retention (Drori et al. 1995; Mark et al. 2001).

The other excipient mostly described in the literature is ethanol, a substance whose presence must be indicated on the drug that contains it.
Cases of alcohol poisoning have been reported in patients receiving high doses of ethanol or in pediatric population, who are more susceptible to the effects of this substance (Zuccotti and Fabiano 2011).
The food and drug administration (FDA) is warning that the chemotherapy drug docetaxel contain ethanol, also known as alcohol, which may cause patients to experience intoxication or feel drunk during and after treatment (U.S. Food and Drug Administration 2014).
The warning was issued after a review of the FDA database and the data in the medical literature about adverse effects, which identified three cases of alcohol intoxication associated with various presentations of docetaxel (Mirza and Mithal 2011). Two incidents occurred during the infusion; one patient developed symptoms within 24 h.
Eight formulations of docetaxel have been identified by the FDA including generic and brand-name products, from several manufacturers.
The alcohol content in each 200-mg dose ranges from a low of 2 g in original drug to a high of 6.4 grams in docetaxel injections manufactured by Pfizer (U.S. Food and Drug Administration 2014).

In Australia, Pfizer’s docetaxel application was withdrawn before a final decision by the country’s regulator. *)
In contrast, the United Kingdom, and the United States have both authorized this generic.


The alcohol content in a dose of docetaxel may affect the central nervous system and should be taken into account for patients in whom alcohol intake should be avoided or minimized, including patients with hepatic impairment.
Docetaxel formulation with the lowest possible alcohol content should be considered for patients who experience adverse reactions (U.S. Food and Drug Administration 2014). Patient may also have religious objections.

Clinically, the Garrido-Siles study (2015) suggested that some skin toxicities of docetaxel may be caused by the excipients used in the different generics.
There seems to be a relationship between the lower content of polysorbate 80 and the lower incidence of severe hypersensitivity reactions.
However, administration of presentations with higher ethanol content is associated with increased incidence of skin toxicity, occasionally severe, near the injection site of the drug.

There was also a significant increase in the incidence of skin toxicity following the administration of the second generic (17.6 vs. 0 %; p = 0.026). New cutaneous toxicities with generic docetaxel were observed, but relationship with content of ethanol or polysorbate 80 was not assessed.

While the results of the study are statistically significant, it is premature to make definitive conclusions on the quality of generic docetaxel from various sources. Nevertheless, the data are supportive of other studies suggesting that the formulation of generic docetaxel is important to consider, as some appear to be increasing host toxicity in a vulnerable patient population.

The present study has some methodological weaknesses. They are the small sample size and the retrospective nature of the study, including heterogeneous baseline demographic and disease characteristics. These include age, sex, stage, disease site and comorbidities which could have induced bias in the case selection or events reporting. Moreover, different chemotherapy regimens and generics were used, with unavailability of information on levels of impurities, levels of docetaxel and the excipients used. These factors should be considered in future studies.

Generic cancer drugs that we can trust - Cancerworld

A recent clinical study on docetaxel, carried out in Canada on a population of breast cancer patients, compared just one generic with the original. It found little difference in adverse events, bar quite a large increase in grade IV febrile neutropenia in the generic group (see Ann Pharmacother 2014, 48:447–455). But this drug is likely to be of high quality, given it is on the Canadian market, which is said to have one of the world’s best bioequivalence inspection regimes.

Also last year, the FDA issued a warning about an adverse effect from docetaxel. The drug is administered in a solvent containing ethanol (alcohol) and has been issued in two vials, one containing the solvent, that are mixed before administration. In 2009, Sanofi introduced a single-formulation already prepared with alcohol, which had the effect of doubling the alcohol content over the two-vial preparation; other generic suppliers of a single-vial docetaxel can have even more – Pfizer’s preparation has over three times more.

This might seem relatively trivial, but the FDA references a letter by two oncologists in the UK who found a male patient receiving palliative care showed symptoms of alcohol intoxication with the new formulation. They point out that alcohol behaves differently when injected rather than drunk, and in cases such as their patient it could render him unfit to drive, which may have led to him rejecting further chemotherapy, and for this patient there was also “a real risk of a relapse in alcoholism”. People may also have religious objections.

In Australia, Pfizer’s docetaxel application was withdrawn before a final decision by the country’s regulator, but it was about to be refused owing to both alcohol and propylene glycol content, and concerns about bioequivalence – in this case the regulator seemed to want a human study despite it being an intravenous drug (Pfizer did submit a study carried out on dogs). In contrast, the UK, which acted as a reference member for most EU countries, and the US have both authorised this generic.

The Advisory Committee on Prescription Medicines (ACPM), having considered the evaluations and the Delegate’s overview, as well as the sponsor’s response to these documents, advised the following:

The PI for Phenytoin Injection (maximum total daily intake of 8.7 g propylene glycol) contains one precautionary statement on propylene glycol-related alcohol-like symptoms. The PI recommends the end-user not to exceed an administration rate of 50 mg active ingredient per minute (equivalent to 0.42 g propylene glycol per minute). This statement is not relevant to Docetaxel-PF, where a maximum dose of 7.5 g propylene glycol is intended to be administered over an hour (equivalent to 0.13 g propylene glycol per minute).

Advisory committee considerations

The ACPM, taking into account the submitted evidence of quality, safety and efficacy agreed with the Delegate that these products have an overall negative benefit-risk profile for the proposed indication.

In making this recommendation the ACPM considered that:

• The evidence does not support the claim that these products are bioequivalent to the reference product, Taxotere, especially in the higher levels of ethanol and presence of propylene glycol in the proposed formulation.

• There are inadequate data on the physicochemical comparison of micelles formed by the proposed products and the innovator product. In addition, the fate of the micelles in vivo has not been determined.

• There remain safety concerns of potential neurological impairment that can be directly attributed to the alcohol and propylene glycol content and with the undefined interaction with the active ingredient.



Outcome

• Overall, there has been an inadequate consideration of the impact of the formulation differences, the toxicity profile and the impact on a range of body systems, including potential neurological impairment and the undefined interaction with the active ingredient.

The application was withdrawn by the sponsor (Pfizer Australia Pty Ltd) on 18 December 2012, before a decision was made.

Nie mam cukrzycy choć miałem hyperglikemię na czczo a i biorę metforminę ale to już jak wiesz z innego powodu =D .

A konteks mojego postu chyba uległ niezamierzonemu zniekształceniu.
Chyba powinno być napisane że zmniejszona ilość wlewów może zmniejszyć / zmniejsza ryzyko pojawienia się neuropatii, do której w efekcie u mnie doszło.
I być może mniejsza ilość wlewów by temu zapobiegła choć można by było tak tylko spekulować nie wiedząc jak będzie. Choć rozważaliśmy że większa ilość wlewów Dx może skutkować przy oczywistej, większej toksyczności lepszym czasem przeżycia.

Jak to było? ... Minister Zdrowia Ostrzega - ryzyko/decyzja należy do Ciebie... =D


Z Twojej kwerendy wynikają ciekawe rzeczy. O ile jest dla mnie jasne, że preparaty generyczne różnią się od oryginalnych zawartością % leku, sposobem uwalniania, ilością wypełniaczy, sposobem degradacji, np. tabletek, a co za tym idzie jakością wchłaniania i potem różnym stężeniem substancji czynnej w krwi, to nie wiedziałem, że w podawanym Docetakselu jest rozpuszczalnik zawierający alkohol - i to tyle alkoholu etylowego.
Ciekawe jak by to wyszło w badaniu alkomatem? Szkoda, że o tym nie wiedziałem. Mam alkomat używany kiedyś sporadycznie aby wiedzieć, że np. po wypiciu 150 ml (ok. 3 kieliszki) czerwonego, wytrawnego wina, po 2 godzinach od konsumpcji moge prowadzić auto. Gdybym wiedział o Dx i alkoholu, to niechybnie zrobiłbym badania testowe :p


Ostatnio czytaliśmy o silnej reakcji uczuleniowej ze wstrząsem (?) anafilaktycznym u chorego z zaawansowaną POChP, po podaniu małej ilości Dx.
Czy przypadkiem nie była to taka reakcja nadwrażliwości na alkohol? Oczywiście spekulacja i trzeba by było robić próby biologiczne, ekspozycyjne na każdy składnik leku, żeby mieć pewność co spowodowało takie działanie - oczywiście niewykonalne i niebezpieczne dla chorego. Ale warto go pewnie uprzedzić, żeby nie spożywał alkoholu.

Że Dx prowadzi do leukopenii/neutropenii, to wiadomo. Sam miałem podawany w czasie terapii Dx czynnik stymulujący tworzenie kolonii granulocytów G-CSF.
Ale z tekstu cytowanego prze Ciebie wynika, że generyki nasilają i to (neutropenię) oraz bardziej uszkadzają wątrobę.

Powiedziałbym filozoficznie, że nie stać nas na "tanie rzeczy". W kontekście zdrowia ma to zupełnie inny wydźwięk niż gdy planujemy kupno pralki, lodówki czy mebli.

Przeczytaj zalinkowane przeze mnie artykuly. W pierwszym czytamy co następuje:

Prediction of Optimal Number of Cycles in Docetaxel Regimen for Patients with mCRPC


Abstract

Although 75 mg/m2 docetaxel (DTX) with 10 mg prednisolone regimen is a gold standard of the first-line chemotherapy for patients with metastatic castration-resistant prostate cancer (mCRPC), optimal number of the cycles remains controversial.
Most guidelines recommend DTX regimen should be terminated up to 10 cycles, but the optimal number is controversial.

We retrospectively analyzed 279 CRPC patients who received DTX regimen.
Patients having more than 10 cycles of DTX had significantly better overall survival (OS) than those having 10 or less, and prediction model of optimal number of cycles in DTX regimen was constructed based on the risk table employing the combination of three factors
- ALP [cutoff 189 IU/L],
- hemoglobin [11.3 g/dL],
and
- age [65 years] at the start of DTX therapy,
and scoring based on the hazard ratio of each risk factor (ALP 4, hemoglobin 2, age 3)
.

The prediction model could effectively predict the probability of the length of DTX therapy, with the c-index of 0.7274.
Since the new drugs are clinically available, optimal number of cycles should be determined based on the PS, toxicities, or patients’ wish.
Our predicting model may help in patients’ consultation before starting DTX regimen, or in selecting potential patients with intolerant DTX regimen.
