Adjuvant and Salvage Radiotherapy after Radical Prostatectomy in Prostate Cancer Patients (2017)


Abstract

Context:
Prostate cancer (PCa) patients found to have adverse pathologic features following radical prostatectomy (RP) are less likely to be cured with surgery alone.

Objective:
To analyze the role of postoperative radiotherapy (RT) in patients with aggressive PCa.

Evidence acquisition:
We performed a systematic literature review of the Medline and EMBASE databases. The search strategy included the terms radical prostatectomy, adjuvant radiotherapy, and salvage radiotherapy, alone or in combination. We limited our search to studies published between January 2009 and August 2016.

Evidence synthesis:
Three randomized trials demonstrated that immediate RT after RP reduces the risk of recurrence in patients with aggressive PCa.
However, immediate postoperative RT is associated with an increased risk of acute and late side effects ranging from 15% to 35% and 2% to 8%, respectively. Retrospective studies support the oncologic efficacy of initial observation followed by salvage RT administered at the first sign of recurrence; however, the impact of this delay on long-term control remains uncertain.
Hopefully, ongoing randomized trials will shed light on the role of adjuvant RT versus observation salvage RT in individuals with adverse features at RP. Accurate patient selection based on clinical characteristics and molecular profile is crucial.
Dose escalation, whole-pelvis RT, novel techniques, and the use of hormonal therapy might improve the outcomes of postoperative RT.

Conclusions:
Immediate RT reduces the risk of recurrence after RP in patients with aggressive disease. However, this approach is associated with an increase in the incidence of short- and long-term side effects.
Observation followed by salvage RT administered at the first sign of recurrence might be associated with durable cancer control, but prospective randomized comparison with adjuvant RT is still awaited.
Dose escalation, refinements in the technique, and the concomitant use of hormonal therapies might improve outcomes of patients undergoing postoperative RT.


Patient summary:
Postoperative radiotherapy has an impact on oncologic outcomes in patients with aggressive disease characteristics.
Salvage radiotherapy administered at the first sign of recurrence might be associated with durable cancer control in selected patients but might compromise cure in others.

4. Wnioski
Prostatektomia ratującą dla ściśle wybranych pacjentów jest skutecznym sposobem leczenia. Częstotliwość powikłań została zredukowana w ciągu ostatnich lat przez udoskonalenie technik operacyjnych i radioterapeutycznych. Jednakże zwężenia zespoleń pęcherzowo-cewkowych i nietrzymanie moczu pozostają problemem dla jednej trzeciej pacjentów.
Pacjenci z podejrzeniem wznowy miejscowej, z przewidywanym długim czasem przeżycia, długim czasem podwojenia PSA oraz PSA <10 ng/ml są dobrymi kandydatami do RP ratującej.

Krioablacja
ratująca jest metodą alternatywną w leczeniu PCa. Wyniki leczenia tą metodą są gorsze w porównaniu z RP ratującą. Możliwymi komplikacjami tej metody są: zwężenie cewki, nietrzy-manie moczu, przetoki oraz przewlekły ból.

Doświadczenia z brachyterapią ratującą są ograniczone, dlatego metoda ta nie jest obecnie rekomendowana.


Konformalna RT loży po sterczu w przypadku wznowy PSA i potwierdzonej biopsją wznowy miejscowej PCa po RP pozostaje jedyną potencjalnie leczniczą metodą.
U pacjentów z wznową biochemiczną po RP i z wysokim prawdopodobieństwem wznowy miejscowej (dodatnie marginesy chirurgiczne, brak powiększonych węzłów chłonnych, brak infiltracji pęcherzyków nasiennych, długi czas do wzrostu PSA i długi czas podwojenia PSA) - zalecana jest wczesna RT ratująca.
Zapewnia ona trwałą kontrolę biochemiczną u 17-78% pacjentów.
Wskaźnik całkowitego prze-życia wynosi 82-98%.
Jak dotychczas nie jest potwierdzona korzyść z dodatkowego leczenia hormonalnego.

Wczesne rozpoczęcie leczenia przez RT ratującą wydaje się być korzystniejszym postępowaniem niż rozpoczęcie leczenia po potwierdzeniu wznowy miejscowej przez biopsję.

Dalsze prospektywne badania kliniczne są niezbędne, by określić dokładnie rolę radioterapii ratującej, szczególnie w porównaniu z adjuwantową radioterapią.

CONCLUSIONS:
In this retrospective study, cancer control after eSRT greatly depended on pretreatment PSA.
The absolute PSA level had a different prognostic value depending on the pathologic characteristics of the tumour.
In patients with more adverse pathologic features, eSRT conferred better cancer control when administered at the very first sign of PSA rise.
Conversely, the benefit of eSRT was less evident in men with favourable disease at RP.




PATIENT SUMMARY:
In this retrospective study, cancer control after early salvage radiation therapy (eSRT) was influenced by pretreatment prostate-specific antigen (PSA) level.

This effect was highest in men with at least two of the following pathologic features: pT3b/pT4 disease, pathologic Gleason score ≥ 8, and negative surgical margins. In these patients, eSRT conferred better cancer control when administered at the very first sign of PSA rise.

Executive summary

=> For prostate cancer patients with adverse pathological features after radical prostatectomy, the two treatment alternatives are adjuvant radiation therapy (ART) versus observation with delayed use of salvage radiation therapy (SRT) for a rising prostate-specific antigen blood test.

=> A recent decision analysis model suggested that observation with SRT was preferred over ART.

=> The development of genomic classifier testing, which provides individualized estimates of cancer recurrence, offers the possibility of considering a patient's personal risk of recurrence when making a decision regarding ART.

=> We designed a Markov decision analysis model to represent the clinical alternatives of ART versus observation with selective SRT over a 10-year time horizon.

=> Sensitivity analyses were performed using a broad range of potential values to represent heterogeneity in genomic classifier-based estimates of cancer recurrence and other model input estimates.

=> On average, observation with SRT resulted in more quality-adjusted life years than observation.

=> In sensitivity analyses, the higher estimate for genomic classifier score resulted in ART being preferred over observation.
