Wiadomość o tym badaniu klinicznym CHAARTED wydaje mi się bardzo ważna, bo zmienia naszą wiedzę o standardowym leczeniu zaawansowanego CAP czyli nie jak dotychczas HT i CHT gdy rak stanie się hormonoodporny lecz jednocześnie HT i CHT. Ta nowa terapia przedłuża OS o 13 miesięcy.

Adding Chemotherapy to Hormone Therapy Improved Survival in Men with Newly Metastatic Prostate Cancer

Starting chemotherapy along with hormone therapy in men with newly diagnosed hormone-sensitive prostate cancer improved overall survival (OS) by more than 13 months in comparison with hormone therapy alone, a phase III study found (Abstract LBA2). The survival benefit was even greater in men with high-volume disease.

“This is one of the biggest improvements in survival we have seen in a trial involving patients with an adult metastatic solid tumor,” Christopher Sweeney, MBBS, of the Dana-Farber Cancer Institute, said. “The certainty of the data is strong for patients with a high volume of metastatic disease and clearly justifies the treatment burden.”

At Sunday’s Plenary Session, Dr. Sweeney presented results of the E3805 trial, titled Chemohormonal Therapy versus Androgen Ablation Randomized Trial for Extensive Disease in Prostate Cancer (CHAARTED).

The trial compared “upfront” chemotherapy plus androgen deprivation therapy (ADT) to ADT alone in men with metastatic prostate cancer.
Between July 2006 and November 2012, 790 men were randomly assigned to a maximum of six cycles of docetaxel plus ADT or ADT alone. Chemotherapy could be given at the investigator’s discretion upon evidence of disease progression.
At enrollment, patients were stratified by extent of metastatic disease as high-volume or low-volume;
high volume was defined as visceral metastasis and/or four or more bone metastases
.

The original design of E3805 called for enrollment only of patients with high-volume metastasis, but the design was amended to include low-volume disease because of slow accrual. The primary endpoint was OS, and secondary endpoints included time to progressive disease (PD) and time to symptomatic PD.

A planned interim analysis in October 2013, at 53% information, met the criteria for significance and release of the study data. As of January 2014, with a median follow-up of 29 months, there were 101 deaths in the ADT plus docetaxel arm, compared with 136 deaths in the ADT arm.

Patient characteristics were well balanced between the two study arms; notably, approximately two-thirds of patients in each arm were classified as having high-volume metastasis. Approximately three-quarters of men in each arm had received no prior localized treatment.

Median OS was 57.6 months in the ADT plus docetaxel arm and 44.0 months in the ADT arm (hazard ratio
0.61, 95% CI [0.47, 0.80]; p = 0.0003). In men with high-volume disease, median OS was 49.2 months with docetaxel plus ADT compared with 32.2 months with ADT, a difference of 17 months (HR 0.60, 95% CI [0.45, 0.81]; p = 0.0006). In men with low-volume disease, median OS had not been reached at the time of the analysis.

The regimen of docetaxel plus ADT was beneficial in all subgroups analyzed. Median time to clinical progression was greater in the docetaxel plus ADT arm (32.7 months) than the ADT-only arm (19.8 months; HR 0.49, 95% CI [0.37, 0.65]; p < 0.0001). Similarly, the median time to castration-resistant prostate cancer was 20.7 months in the docetaxel plus ADT arm compared with 14.7 in the ADT-only arm (HR 0.56, 95% CI [0.44, 0.70]; p < 0.0001).

Regarding toxicity, 6% of men receiving the chemohormonal regimen experienced febrile neutropenia, 1% experienced significant effects on sensory nerves and 1% on motor nerves, and
1 of the 397 patients who received early docetaxel died as a result of treatment. The cause of death was prostate cancer in 84 patients in the ADT plus docetaxel arm (83.2%) and 112 patients in the ADT-alone arm (83.6%).



Discussion

Discussant Michael J. Morris, MD, of Memorial Sloan Kettering Cancer Center, said, “The investigators have adequately shown that patients with high-volume, castration-sensitive metabolic disease do benefit from upfront docetaxel, and it appears to confer a survival benefit that is superior to docetaxel given at metastatic castration-resistant disease.”

However, he said, there is insufficient evidence with a median 29 months of follow-up to recommend that low-volume patients with castration-sensitive disease undergo early docetaxel therapy, and there is a need to optimize the distinction between those who benefit from chemotherapy and those who do not.

The investigators showed, he said, that “by moving docetaxel from the castration-resistant stage, when men are not responding to testosterone-lowering agents, to the castration-sensitive state, when they are, the survival benefits are amplified, from 2 to 3 months to 13.6 months.”

Dr. Morris noted that, compared with other drug regimens that have been shown to prolong OS in castration-resistant prostate cancer, “none even come close, in terms of survival prolongation,” to the 17 months seen in patients with high-volume metastatic disease. “The best therapies reported previously have less than a third of the benefit seen in [these] patients,” he said.

In the population of patients with low-volume disease, in which 45 deaths occurred in the course of the study, the difference between the two treatment arms was too small to constitute a trend, he said.

Dr. Morris pointed out that those patients with metastatic prostate cancer at presentation “represent only about 4.2% of the overall prostate cancer population, and high-volume disease represents a fraction of that,” but, nonetheless, the results of this study have, “changed the standard of care for some patients, at least.”

Poniżej cytuję (tlumaczenie moje) post jednego z pacjentów amerykańskiego forum HealingWell u którego zastosowano metodę HT+ChT.



Ponad rok temu zdiagnozowano u mnie zaawansowanego raka gruczołu krokowego z przerzutami, o wartości PSA ponad 100 w chwili rozpoznania.Rak prostaty rozprzestrzenił się,w obu płucach.

Jestem w pierwszej fali pacjentów, którzy zostali już leczeni nowo przyjętą metodą polegającą na dodaniu chemioterapii (Taxotere/Docetaxel) do terapii hormonalnej (Lupron) na zaawansowanego raka prostaty, która stała się nowym standardem opieki medycznej w ostatnich miesiącach.

Są jeszcze lekarze którzy nie wiedzą o tej nowej metodzie leczenia, więc być może trzeba znaleźć w necie potrzebną literaturę i przedstawić ją lekarzowi. .......
Właśnie zakończyłem niedawno całą serię wlewów chemioterapii, a hormon LUPRON® będę miał nadal. W ubiegłym tygodniu, mój onkolog poinformował, że mój PSA jest teraz mniej niż 0,05, za co jestem niezmiernie wdzięczny. Idąc od początkowej wartości PSA ponad 100 w dół do wartości PSA poniżej 0,05 wydaje się cudem, dla kogoś w mojej sytuacji.
